Required - Gameday Wellness Survey

We all play a role in reducing the spread of COVID-19. ALL attendees are REQUIRED to complete a Gameday Wellness survey no more than 24 hours prior to entering Comerica Park. If minors are in your party, you can complete the form on their behalf.

We recommend completing the survey within the MLB Ballpark app, which is also where you can access your game tickets.

This survey can be completed within the MLB Ballpark app.

You will receive prompt confirmation upon completion of your survey that you will need to show, along with your mobile ticket, to enter the park. We recommend you take a screenshot of your survey results for easier access. Do not take a screenshot of your digital ticket. Screenshots of tickets will NOT work.
